Adoption Application

Download and open the application in Microsoft Word. If the application opens in your web browser, simply select File from the main menu and then Save As from the flip-down menu to save it to your hard drive.

Complete the application and then attach it to an email to gingerluke@comcast.net with the dog's name as the Subject.

We also need you to email us a photo of:
  • Front of your home
  • Back of your home showing fenced in back yard & for any dog under 25 lbs you are not required to have a fenced in yard.
  • Photo of your home address on the house
  • Copy of a utility bill showing your name & home address

Before we introduce you to one of our dogs, we need to review your application to make sure the dog will fit your home & environment.

 Terms Of Adoption/Foster

 

1)      To keep this dog in my personal possession, and to provide proper and sufficient food, water, shelter, grooming and humane treatment at all times.

 

2)      To procure veterinary care at once if this dog becomes sick or injured, and to keep current all vaccinations as recommended by my veterinarian.

 

3)      To obey any and all animal control regulations governing the area in which I live, and to license this dog according to such regulations within one month of adoption.

 

4)      Not to sell, trade ownership, abandon, or dispose of this dog in any way, but to notify Ginger’s Pet Rescue if I must relinquish custody of the dog. This includes release to family members.

 

5)      To allow Ginger’s Pet Rescue to examine the dog and it’s living conditions, and to surrender it to said person for return if the conditions are found unsatisfactory.

 

6)      To assume full responsibility for this dog actions, including any damage done by this dog.

 

7)      I will microchip the dog within 30 days of adoption and mail proof to Ginger’s Pet Rescue 

      (If adopted only and not already done)

 

8)      I agree to have on the adoptive dog microchip registration for the life of the animal.

 

All the information I have given above is true and complete. I have also read and agree with the Terms of Adoption/foster. This dog will reside in my home as a pet. I will provide it with adequate food, water, shelter, training, affection and medical care. I also understand I have 30 days from the date of adoption only to microchip him/her. (If not already done) I will need to mail proof it has been done to Ginger’s Pet Rescue. I also agree to put Ginger’s Pet Rescue as a contact person for the life of my new dog on his/her microchip registration info.  I understand the temperament; health, habits and physical condition can’t be guaranteed. I understand that it is my responsibility to see and evaluate the dog for myself before agreeing to adopt it. I am in full agreement with these terms of adoption. Ginger’s Pet Rescue is in no way liable or responsible for any damage, accident or injury resulting from the placement of a dog into my household.
